酗酒会伤脑子

针对酒精摄入的长期影响所进行的研究中,规模最大的其中之一显示,酗酒与中年男性认知能力的显著衰退有关。不过,看起来女性的大脑出于一些尚不明确的原因而免受其害。该研究发表于《神经病学》期刊( Neurology)。

在这项研究中认知能力衰退是从两方面来界定的:一是执行能力减退(大脑的处理速度和效率),二是记忆力衰退。

研究人员对Whitehall II定群研究的数据进行了分析,该研究始于上世纪八十年代中期,样本是约10,000名英国公务员——他们同意完成关于生活方式的问卷调查并在近20年的时间跨度中于几个特定时间接受身体检查。该研究设计使得研究人员可以追踪如饮酒等生活方式的长期影响。

该研究发现,十年来每天摄入36克酒精——相当于1.2安士(fl.oz,一安士约为30毫升——译注)——或者更多的中年男性(平均年龄56岁),记忆力以及执行能力的减退程度大于“偶尔”或“适度”饮酒者,这些人每天的酒精摄入量在0.03至1.1安士之间。

换种角度来看,一口杯(shot)80度的威士忌是1.5安士,其中只有0.6安士是纯酒精。因此,按照该研究的定义,酗酒者指的是那些每天至少喝上两口杯的人。如果研究对象喝的是葡萄酒或啤酒,他们每天至少喝10安士葡萄酒或24安士啤酒才能达到相同的酒精摄入水平。

酒的种类对研究结果没有影响;从啤酒、葡萄酒或烈酒中摄取1.2安士或以上的酒精会得到同样的结果。

鉴于两性在酒精代谢方面存在差异,对女性酗酒者的定义有所不同。达到本研究中的酗酒水平,女性需每天至少摄入19克酒精(0.6安士)——约为男性标准的一半。

奇怪的是,女性研究对象中的酗酒者没有出现近似于男性的认知衰退程度。

该研究的作者们没有就这一差异的原因进行推测,但有可能是荷尔蒙——也就是雌激素——在起作用。2001年加州大学戴维斯分校的研究者们所做的一项研究显示,雌激素充当了大脑细胞健康的卫士,荷尔蒙水平升高与阿尔茨海默症患病风险降低之间存在联系。雌激素可能也充当了抵御酒精中毒的荷尔蒙屏障。

值得注意的是,尽管该研究显示女性大脑没有出现同等程度的酒精中毒现象,但还有其他大量理由告诫女性要适度饮酒,其中最大的一个因素就是出于肝脏健康考虑。

像任何定群研究一样,本研究也存在一些弊端——其中最主要的是:长期影响大脑健康的因素有很多,很难孤立出某些特定因素(如酒精摄取),把它们作为认知能力衰退的罪魁祸首。

另一方面,该研究的优势在于数字。研究中大约10%的男性被认定酗酒(超过700人),而且这些人中认知能力衰退的迹象一直很明显。

偶尔和适度饮酒的人,他们的大脑健康状况经过这么多年后看起来非常良好,戒酒或压根不喝酒的人也同样如此。

Alcohol consumption and cognitive decline in early old age

Objective: To examine the association between alcohol consumption in midlife and subsequent cognitive decline. Methods: Data are from 5,054 men and 2,099 women from the Whitehall II cohort study with a mean age of 56 years (range 44–69 years) at first cognitive assessment. Alcohol consumption was assessed 3 times in the 10 years preceding the first cognitive assessment (1997–1999). Cognitive tests were repeated in 2002–2004 and 2007–2009. The cognitive test battery included 4 tests assessing memory and executive function; a global cognitive score summarized performances across these tests. Linear mixed models were used to assess the association between alcohol consumption and cognitive decline, expressed as z scores (mean = 0, SD = 1). Results: In men, there were no differences in cognitive decline among alcohol abstainers, quitters, and light or moderate alcohol drinkers (<20 g/d). However, alcohol consumption ≥36 g/d was associated with faster decline in all cognitive domains compared with consumption between 0.1 and 19.9 g/d: mean difference (95% confidence interval) in 10-year decline in the global cognitive score = −0.10 (−0.16, −0.04), executive function = −0.06 (−0.12, 0.00), and memory = −0.16 (−0.26, −0.05). In women, compared with those drinking 0.1 to 9.9 g/d of alcohol, 10-year abstainers showed faster decline in the global cognitive score (−0.21 [−0.37, −0.04]) and executive function (−0.17 [−0.32, −0.01]).
